The first time you submit a Picture Tour please fill out this form with all your basic information that we will need for our records and the contact information that will be used on each of your Picture Tours/Flyer Pages.

You only fill out this form once (unless you need to make changes to your basic information or contact information).

This is the form you use each time you submit a picture tour. If you email us your pictures (the preferred method), please submit the form at the same time you submit the pictures. If you are not sure how to attach pictures to an email please contact us and we will give you step-by-step instructions.

If you send us the pictures on diskette or as photos please print out the Form and send it along with the pictures. Do NOT email us the form separately as it will create confusion. Thanks.
